Self-pay & Superbills

We are an out-of-network provider, which means you may file a claim for reimbursement with your insurance company. At every appointment the credit card on file will be charged for the full cost of the visit and an email receipt will be sent. You will also receive a “superbill” after the appointment is over. Superbills contain the information insurance companies need, like CPT codes, diagnostic codes, amounts billed, practice and provider NPI numbers, and the practice’s full address and tax identification number. This information is typically required to process claims for policy holders who self-pay. We encourage you to call your insurance company to confirm their policy and process for filing out-of-network provider claims. Certain services, especially those considered by your insurance provider to be elective, may not be covered by insurance. So, it is important to verify specific out-of-network coverage with your insurance plan before receiving treatment.
